Rebate for FS2002 in Canada

I am trying to get the rebate for FS2002 in Canada. I sent in my rebate
to this address:

P.O. Box 641
Fore Erie, On L2A 6L8

But it was returned as address unknown.
Does anyone know what the correct address is?

The coupon said that the rebate was good until September30, 2005, so it
should be still valid.
